# Find the pharmacy behind the medicine

Ask for the pharmacy’s full name before judging a delivery offer. The compounder is the pharmacy or doctor who makes a medicine to order. Ask who prepares it and who sends it.

Check the pharmacy by its own name. Keep that name with the medicine details.

## Complete the pharmacy check

- Get the full name of the pharmacy and any separate compounder. 

- Ask for the pharmacy’s Arizona permit details. 

- Ask how to reach a pharmacist with product questions. 

- Match the pharmacy named in the plan with the name on delivery paperwork. 

## The permit and the product are separate

Choose Arizona in that list. Ask the board for help if the name or permit type cannot be matched.
