# Keep the whole care checklist

Print this page or keep it open during the conversation. Tick a line when the answer is written down; leave open lines for the next discussion.

## Answers in hand versus answers still open

### In hand

The exact medicine, the people responsible, the pharmacy, the ongoing bill, and a way to get help once the visit ends.

### Still open

Mark the question to ask next. Add the name or role of the person who can answer it.

The marks record answers gathered. They do not make a treatment decision.

## The printable care check

- Medicine: exact name, shot or tablet, and treatment reason written down.

- Prescriber: full name, role, and licence match checked.

- Video visit: prescriber’s licence or other board permission saved and checked for where I will be during the visit.

- Health check: questions, records, tests, and any office visit discussed.

- Pharmacy: the place that makes the medicine and the pharmacy that fills and sends the prescription named. If they are the same, one name is enough.

- Permits: Arizona pharmacy permit details matched.

- Made-to-order medicine: the reason it is being made for me explained.

- Cost: ongoing total and extra charges written down.

- Insurance: exact medicine and proposed use checked with the insurer.

- Payment: offer end date, renewal, and cancellation terms saved.

- Medicine information: label, storage instructions, and pharmacist’s phone number or message address available.

- After the visit: phone number or message address for the person treating me, and the plan for reviewing treatment, written down.

## The checks that stay separate

A pharmacy permit does not establish that each compounded (made-to-order) product offered is allowed, as of October 3, 2026. [Arizona rules · Product and permit ↗ ](https://apps.azsos.gov/public_services/Title_04/4-23.pdf)

Compounded medicines have no FDA (Food and Drug Administration) approval and receive no FDA review for safety, quality, or results before sale, as of September 26, 2026. [FDA · Compounding limits ↗ ](https://www.fda.gov/drugs/human-drug-compounding/compounding-and-fda-questions-and-answers)

Use the open lines to guide the next conversation. Keep the written answers with the visit record.

Peoria care checklist helps readers prepare care questions, and its role does not include running a clinic or selling medicines.
