# Ask your insurer to check the named medicine and its use

Check the medicine name, why it is prescribed, and your health insurer’s current rules. Ask what you would pay after the insurer decides how much it will cover.

## A possible benefit versus a confirmed payment

### A possible benefit

A medicine appears in your insurer’s drug list. Ask which uses it covers and what permission is needed before it pays.

### A confirmed payment

Ask for the decision, any conditions, and your expected share of the bill.

Keep the medicine name and the treatment reason beside the insurance answer.

## Ask the insurer

- Is this exact product covered for the proposed use?

- Is prior authorization needed: the insurer’s permission before it pays?

- What information must the prescriber submit?

- What is my share after any deductible, the amount I pay before the insurer begins paying?

- How is a denied request reviewed?

## Medicare has different payment rules

Ordinary Medicare drug coverage does not pay for medicines used for weight loss, as of September 26, 2026. [CMS · Drug benefit exclusions ↗ ](https://www.cms.gov/medicare/prescription-drug-coverage/prescriptiondrugcovcontra/downloads/partddrugspartdexcludeddrugs.pdf)

Some other uses, such as treating type 2 diabetes, can be covered. Your Medicare drug plan’s medicine list and any permission needed before it pays still apply. [CMS · Coverage by treatment use ↗ ](https://www.cms.gov/newsroom/fact-sheets/contract-year-2026-policy-and-technical-changes-medicare-advantage-program-medicare-prescription)

GLP-1 is a natural body signal that helps manage hunger and blood sugar. Medicines that copy this signal include semaglutide and tirzepatide.

At the September 26, 2026 check, the temporary Medicare GLP-1 Bridge offered eligible patients certain medicines at a $50 monthly copay (patient payment). [CMS · Temporary Medicare bridge ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge)

This programme begins July 1, 2026, with December 31, 2027 as its end date. It uses a separate payment system rather than your usual Medicare drug plan’s payment process. In 2026, one central service handles permission requests and payments to pharmacies. [CMS · Programme dates and payment ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge)

Ask your prescriber and pharmacist how to submit a Bridge request through that service.

The rules checked September 26, 2026 require a prescription and prior authorization, meaning permission before the programme pays. You must be 18 or older and enrolled in a qualifying Medicare drug plan. The main options are a separate Medicare drug plan and a Medicare Advantage plan combining care with drug coverage. Treatment must also include ongoing changes to food and physical activity. [CMS · Who qualifies ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-providers)

Ask your insurer to check your plan type against the linked rules before assuming you qualify.

Your height and weight are used to calculate BMI, and the weight rules use the value recorded when GLP-1 treatment started. A BMI of 35 or above meets the weight rule without another health problem. [CMS · Weight rules ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-providers)

Another way needs BMI 30 or above and one of the following health problems. These are heart failure with a normal heart pumping measurement, high blood pressure that is hard to control, or kidney disease at stage 3a or later. A kidney-disease stage is a medical rating of how well the kidneys work. For the blood-pressure rule, the top number must be above 140 or the bottom above 90 despite two blood-pressure medicines. [CMS · Heart and kidney rules ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-providers)

Another route requires BMI 27 or above plus pre-diabetes (raised blood sugar before diabetes), an earlier stroke or heart attack, or artery disease outside the heart that causes symptoms. [CMS · Other health rules ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-providers)

Ask the prescriber to check your height, weight, and exact diagnosis against these rules.

Type 2 diabetes and moderate or severe sleep apnea (breathing pauses during sleep) rule out the Bridge. So does the liver disease called MASH when it has moderate or advanced scarring but has not reached cirrhosis, a late stage of liver scarring. Those treatment uses go through the usual Medicare drug-plan rules instead. Having any fatty liver diagnosis does not by itself rule out the Bridge. [CMS · Uses that follow usual coverage rules ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-providers)

If a Medicare drug plan paid for a GLP-1 medicine for you during 2026, the Bridge is unavailable to you for 2026. [CMS · Earlier payment rule ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-providers)

The covered products are orforglipron (Foundayo) tablets, semaglutide (Wegovy) shots or tablets, and tirzepatide (Zepbound) in the KwikPen device. Zepbound single-dose vials and single-dose pens are excluded. [CMS · Covered products ↗ ](https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-providers)

The Bridge’s $50 monthly payment does not count toward your usual Medicare drug plan’s yearly spending limit for covered medicines. It does not help you reach the point where that plan stops charging you for covered drugs for the rest of the year. [Medicare · How the payment is counted ↗ ](https://www.medicare.gov/coverage/weight-loss-drugs)

## Keep this answer

Keep a dated coverage answer for the exact medicine and use. A broad “covered” claim leaves too much open.

Peoria care checklist helps readers prepare care questions, and its role does not include running a clinic or selling medicines.
