Ask who reads it, who can answer a medicine question, and how a question reaches that person.

Compare the reply process before it is needed. Record names or roles, rather than a broad service claim.

## Get the follow-up plan

- Who reviews any test result, and how do I receive it? 

- How do I send a question to the prescriber? 

- How do I ask the pharmacist about the product or label? 

- What response time is offered? 

- When is the treatment reviewed? 

- What happens if an in-person examination is needed? 

- How do I obtain and share my records?
